Rehabilitation is provided after acute care is completed and is usually related to a specific disease or diagnosis such as a heart attack, stroke or spinal cord injury. Types of rehab programs include:

Rehabilitation is an important part of the recovery process for many different illnesses, injuries and surgeries that result in physical or neurological impairments . For example, someone may be experiencing limited or painful movement of hands, arms, legs or other parts of the body. They may be having difficulty with physical activities like sitting up, standing or walking. Balance may be a problem. Or it could be problems with breathing, swallowing or talking.

Rehabilitation is a program that helps a person who is recovering from illness or injury to regain as much function as possible. It is a process of helping a person achieve the highest level of function, independence, and quality of life possible . Rehabilitation is intended to help an individual recover as fully as possible and, where full recovery is not possible, to function with any limitations as independently as possible. Rehabilitation also aims to teach strategies for ongoing disabilities.

Rehab is now being delivered in a wide variety of settings, acute inpatient, as an outpatient and in the community:

Inpatient – for patients admitted to a hospital bed. Rehabilitation may take
place in a community, acute teaching or rehabilitation hospital.
Outpatient – for patients who can travel to the hospital for each
rehabilitation session.
Home/community – when the rehabilitation professional comes to the
patient’s home/place of residence, usually because the patient cannot
travel to the hospital or outpatient clinic.

Patients may receive rehabilitation in more than one setting during the treatment process. For example, they may begin with inpatient services during a hospital stay and then transfer to outpatient or home-based services following discharge from hospital.

The Rehabilitation Team consists of a number of professionals who are trained in different areas of rehabilitation. You may need to see one or all of these people.

Audiologists

Home care workers
Social workers
Pharmacists
Orthotic and prosthetic professionals
Dietitians

In all cases, patients and their families are actively involved with the health professionals in setting and working toward the goals for rehabilitation.
